Workflow for Avicenna Health and Home Health agencies and assisted living facilities collaboration

If you have a patient with no primary care physician (PCP) (or no available PCP to address the plan of care oversight); Avicenna Health is pleased to collaborate with your agency to take care of the patient at home virtually. For a referral, please follow the following:


  1. Please email the patient's demographic page, insurance information, and the date of the next planned home health visit, the available medical and clinical records to Alicia Breedlove. 2-3 business days notice in advanced is appreciated.
  2. The office staff will check the patient's eligibility to be seen by Avicenna Health and email back with the response.
  3. Once the patient's eligibility for a virtual visit is verified, our staff will confirm the appointment with Avicenna Health Provider.
  4. The patient should review the following documents and sign them before the virtual visit. The nursing staff's help is appreciated.
  5. A smartphone number with internet is needed for the F2F virtual visit (HIPAA-secure link will be sent through SMS to start the video visit). If the patient does not have a smartphone, the staff smartphone should be used. Ideally, the virtual visit should begin after the nursing staff perform a physical exam and vital signs check.

For Long-Term Facilities:

We recognize that the healthcare provider may not be available all the time at the Long-term facility (e.g. nursing home, skilled nursing facilities, or mental health facility) to evaluate and treat the residents when there is a change in their usual state of health. When the provider is away, relying on the phone calls by staff may not be the optimal option. Due to the lack of optimal evaluation over the phone, residents are potentially transferred to the nearest ED for evaluation. Whether if the patient is admitted to the hospital or transferred back to the long-term facility, the transfer process has a toll on the patient, the staff, and on the available resources and healthcare costs.

This goal is not new but comes with more scrutiny and cost now than before as CMS continues to commit for shifting from a volume-based payment model to value-based payment models.

Most patients with psychiatric diagnoses have several medical problems that need closer monitoring and medication management during behavioral and mental health hospitalizations. We recognize that managing these medical problems by psychiatrists and psychologists is challenging.
