Can I obtain a Disability Parking Placard from Avicenna Health?

Avicenna Health is here to help you get your placard with convenience from the comfort of your home without the need for unnecessary physician office visits.

How does it work?

After creating an account on Avicenna health platform and making the initial payment required, we will schedule you for a HIPAA compliant video telemedicine visit with the provider who is going to discuss your disability concern.

Make your initial payment.
Schedule a virtual visit with an Avicenna Health provider.
Avicenna Health provider will conduct a HIPAA compliant video telemedicine visit and then fill and sign the DMV form for your state
Your signed DMV form will be sent to you by email and uploaded to your Avicenna Health patient portal as well for you to download.

How long does Avicenna Health take for the forms to be completed and emailed back?

After your televisit, the Avicenna Health provider will determine if further records from your primary care physician or specialists are needed.

We should provide you with a completed Parking Placard form within 24 hours, if no further records are needed.

If records are requested, we will notify you by email that your records are received and we should provide you with a completed Parking Placard form within five business days after receiving your records.

Have a disability (Physical, respiratory, vision, obesity, etc.)

The total cost for the service is $99 (including the telemedicine visit fee of $65 and $34 processing fee).

(A refund for the processing fee if your Handicap permit is not approved by your Avicenna Health Provider)

Once your registration is completed, you will receive an email with the appointment confirmation. At the time of your appointment, you will receive a text message to your smartphone with a link that connect you directly to our virtual doctors for a video consult. For best video and consultation quality, please connect to a Wi-Fi.

If you prefer to use your laptop or a desktop for your video visit, please follow the instructions in your email.

LAB TESTING

Patients using Avicenna will be offered the option to get an electronic lab slip order to be done at their laboratory facility of choice. These options may include Labcorp, Quest Diagnostics or any other preferred facility of choice. Once the results are received and uploaded in our system, you will have the option to review the lab results on your patient portal and schedule a virtual follow up appointment to discuss results with your provider.

IMAGING STUDIES

Avicenna Health provider may offer the patient the option to get imaging study ordered such as ultrasound, X-ray, or a CT scan to be done at the facility of choice. Once the results are received and uploaded in our system, you will have the option to review the results on your patient portal and schedule a virtual follow up appointment to discuss the results with your provider.
